- Includes $89 for a $1000 discount applicable on the total price of Advanced Custom Wavefront LASIK, PRK, or All-Laser LASIK on both eyes at LASIK MD Halifax location only.
- The cost of the Advanced Custom Wavefront LASIK procedure ranges from $1750 to $2190 per eye (between now and promotional value expiration date). Prices are subject to change without prior notice after December 31, 2012 expiry date. Please inquire for more details.
- Includes pre-operative consultation (complete eye exam) to determine candidacy. Laser eye surgery is subject to medical qualification as determined by a licensed professional of the Merchant in his/her sole discretion. Licensed professional of the merchant will determine, at his/her sole discretion, if patient is a candidate. A full refund in the original form of payment of the amount paid ($89 for 1 ticket) will be issued by TeamBuy only if patient is deemed a non-candidate.
- Must schedule a free pre-operative consultation. Pre-operative consultation and surgery have to be completed by 31/12/2012. Tickets that are not redeemed prior to 31/12/2012 revert to their face value of $89.
- Ticket is transferable in its full amount (a ticket can only be used by 1 person and cannot be shared between 2 patients). Financing is available on the remaining balance. Inquire with LASIK MD at the time of consultation.
- Ticket valid from the date the TeamBuy goes live (27/08/2012) and is not valid towards previous surgery or previously scheduled procedures. Eye drops are not included (typically costs $60).
- Entire value of the ticket must be used in a single visit and cannot be combined with any other discount, special offer, corporate plan or special financing offers.
- The procedure is non-taxable.
